Case Study Navigator Group Valueworks In this study we explored the delivery and implementation of a community health navigator (chn) intervention. the core function of patient navigation is to eliminate barriers to timely care and promotion of patient movement through a complex system of care [16]. This work highlights the transformative influence of human navigators in care systems, demonstrating how community resource advocates and consensus builders enhance care access and interorganizational response, creating more equitable service delivery.
Navigator Community Our Community The following case study illustrates the navigator role in providing low threshold, harm reduction navigation services and community linkages to increase super utilizers' access to healthcare and reduce unnecessary hospital utilization. In this case study, we describe the development and implementation of a model utilizing student volunteer community resource navigators to help patients connect with community based. Established through private donor funding, the scnp aids older adult patients (and their caregivers) struggling to negotiate health and social care systems at hfh and in its surrounding communities, by offering support via navigation, information, advocacy, and access to resources.

In this case study, an oncology nurse navigator in a university based medical center is contacted by a member of the community, “bb,” regarding the institution’s lung cancer screening program. This manual was written to guide the community navigators who were delivering the programme within the context of the research study. it is designed to be of general use for future community navigators to deliver the programme elsewhere. The coproduced intervention, developed in this study, includes up to ten sessions with a ‘community navigator’ over a 6 month period. This case study describes key strategies that health quality innovators, an alignment track bridge organization, developed to conduct community service navigation as part of the accountable health communities model.
