Therefore, this study investigated how lhds are utilizing the healthy connecticut 2025 framework to achieve health equity, as well as their successes and challenges in doing so. this study also explored whether differences in health equity efforts exist among lhds in low and high income cities. Yale new haven hospital, recently completed its 2025 community health needs assessment process. this included the development of a hospital implementation strategy plan to address the major unmet needs in the region. New haven’s population increased 0.3 percent. with a growth rate of 3.3 percent, new haven was among the fastest growing places in the state, w th a net gain of 4,244 residents (see table 2a). compared to connecticut, greater new haven has the same share of latino residents (both 17 percent) and a similar share of for. The pilot project is being implemented in the cities new haven, hartford, and groton with a goal of eventually being incorporated into local health department activities throughout the state and the way all local health departments conduct their business.
