Dementia Journeys To identify gaps in support and opportunities for improvement, we conducted a longitudinal, qualitative study of the experiences of care partners and mapped their care journey over time. The dementia ideal care map summarizes the ecosystem of over 200 best practices, nearly 100 technology enablers, other infrastructure, and enhanced care pathways in one comprehensive diagram.
Dementia Journeys Implementing care navigation can help overcome these barriers by providing vital linkages between health and social care services and the “care dyad”— individuals living with dementia and their caregivers. For this study, we evaluated the long term treatment effects of the care ecosystem, a telephone based collaborative care model for dementia with care navigation among plwds and their caregivers, who were selected for the study based on high caregiver burden at baseline. Family caregivers of persons with dementia (pwd) provide critical and often sustained support across the dementia trajectory. despite growing recognition of their evolving needs, many interventions remain episodic and not tailored to different. This study aimed to map the existing evidence of dementia care pathways and to determine the extent of involvement by persons with dementia and or their care partners in developing, implementing, and evaluating the care pathways.
