Enhancing Your Advocacy Toolbox Protecting Residents From Nursing This resource was developed as part of the ombudsman learning collaborative to protect residents against nursing facility discharges project funded by the administration for community living (acl). seven state ombudsman programs representing six of the ten acl regions participated in the project. This review provides insights into a sample of facility initiated discharges from nursing homes and the extent to which these discharges followed federal requirements.

Protecting Residents From Nursing Facilityinitiated Transfersdischarges Mobilization for justice provides information, advice, advocacy and legal representation for nursing home residents in three areas: involuntary discharges, particularly to shelters; reasonable accommodation requests; and day pass disputes. Ltc ombudsmen advocate for residents of nursing homes, board and care homes, and assisted living facilities. they are trained to assist residents with complaints and resolve problems. ombudsmen also provide information about how to find a facility and what to do to improve the quality of care. The resident advocate is a newsletter for residents of long term care facilities. it provides information on residents’ rights and care issues; news and updates on national policy; and self advocacy tips for obtaining person centered, quality care. The ombudsman cooperates with facility staff and social services to develop comprehensive discharge plans that address every resident’s requirement following their transition. they may suggest further evaluations or require waiting until a well designed care strategy exists.
